Have the following at the appointment for each child:
1) Birth Certificate
2) Social Security Card
3) Most RECENT Immunization Records
3) Student Records from prior school
4) Physical (K-1st Only)
Have the following for yourself as Parent/Guardian
4) Proof of Residency (Utility Bill, Copy of Lease, Rent Receipt, etc.)
5) State ID (Driver's License or ID)
6) Activity Fee: $39.99
